Introduction

Four days training on AHA Certified BLS & ACLS Course for doctors & nurses of Central Referral Hospital was conducted from 23rd April to 26th April, 2023 in level 6 Skill lab SMIMS building. Training was organised by Quality team: Ms Breenila Targain Head Quality, Ms. Bandana Moktan Quality Coordinator (Nursing), Dr. Mala Sharma Quality Coordinator (Operations) Quality Department Central Referral and was facilitated by Life Savers Association New Delhi. The eminent resource persons from Life Savers Association Dr Rakesh Kumar, Dr Anil Misra, Dr Vandana Bagga, Dr Sunil Thakur, Dr Rajender Kumar, Dr Ranju Singh & Dr Unnati Asthana facilitated the AHA Certified course on Basic Life Support and Advanced Cardiovascular Life Support skills. A total of 39 Doctors and Nurses from various disciplines of Central Referral Hospital attended the American Heart Association (AHA) Certified BLS & ACLS Provider course from 23rd to 25th of April 2023 and 06 doctors attended the AHA Certified BLS & ACLS Instructor course on 26th of April 2023. The participants have been awarded with BLS & ACLS Provider E-Card by American Heart Association upon successful completion of the course.

Training Objective:

  • To develop effective BLS & ACLS Skills of doctors and nurses
  • To have certified trainers in house for sustainability and continued training
  • To initiate Code Blue in the hospital

Day 1: BLS Provider Course

Day one started at 8.15 am with an introduction to the team of eminent resource persons and an introductory video of the course. Participants were taken through the sessions on:

  • Recognizing and initiating early management of peri-arrest conditions that may result in cardiac arrest or complicate resuscitation outcome.
  • Demonstrating proficiency in providing BLS care, including prioritizing chest compressions and integrating AED use.
  • Managing cardiac arrest until return of spontaneous circulation (ROSC), termination of resuscitation, or transfer of care.

Each session was conducted through lectures, video session and hands on sessions in the skill station. Participants were divided into groups of 5. AHA standard of 1:5 was followed to conduct skill-based hands-on practice in the 8 BLS skill stations.

Written test and skill assessment for BLS Provider course were conducted at the end of the day. Training concluded at 5.30pm.

Day 2 & Day 3: ACLS Provider Course

The objectives of the ACLS course are to enable the participants to:

  • Identify and treat ischemic chest pain and expedite the care of patients with acute coronary syndromes.
  • Recognize other life-threatening clinical situations, such as stroke, and provide effective initial care and transfer to reduce disability and death.
  • Demonstrate effective communication as a member or leader of a resuscitation team and recognize the impact of team dynamics on overall team performance.

Day 2

Day two started at 8.15am with reflection of day 1. Lectures on ECG and Rhythms for ACLS Providers were conducted till 10am. Thereafter ACLS presentations and Hands-on Workstations on Cardiac arrest algorithms and ACS and Stroke were conducted. Participants were divided into 4 groups in 4 skill stations with 2 instructors each for hands on practice. Training concluded at 6.00pm.

Day 3

Third day started with reflection of day 2 at 8.15 am. Rhythm revision, team dynamics and ACLS Presentations (Bradycardia & Tachycardia) till 10am were conducted. Thereafter ACLS hands on Workstations on Bradycardia and Tachycardia were conducted. Written test and Skill assessment were conducted from 2.00pm till 4pm followed by valedictory session.

Day 4: Instructor's Course

Day 4 started at 9.30 am with reflection of the BLS & ACLS sessions. Six doctors (BLS & ACLS qualified) were chosen for the course of instructor. They were taken through the various topics to be taught by the instructors as per the AHA guidelines. Presentations and workstation on organising and conducting the training were conducted till 12.30 pm thereafter written test was conducted for the instructors followed by wrap up.

Valedictory Session

Valedictory session started from 4.15pm on 25th April. The Honourable Vice Chancellor SMU, Dr (Lt General) Rajan S Grewal graced the valedictory program as a chief guest. The Chief Guest gave an insightful talk on the lifesaving skills and emphasized the need of such trainings to enhance proficiency of doctors and nurses. Dr Murlidhar V Pai, Dean SMIMS, Registrar SMU Dr. K.S Sherpa, MS CRH Dr (Colonel) Navneet Singh Chhabra & Head of the departments also graced the valedictory program. Medical Superintendent CRH, Dr (Colonel) Navneet Singh Chhabra gave welcome speech followed by felicitation of the distinguished resource persons by the Chief guest. Valedictory program concluded with vote of thanks by Dr Mala Sharma Quality Coordinator CRH, SMU.

Evaluation:

Participants were required to score > 75% in the online (AHA site) pre-course assessment before the commencement of the course.

Participants were required to score > 84% in the onsite post course assessment (written test and skill assessment).

Average Pre-course assessment score was 84% and the post course assessment score was 89%.

Feedback:

Feedback was obtained using self-structured google form. Total 30 participants have given their feedback. Findings are as follows:

  • 66.7% of the participants gave excellent rating to the overall structure of the program, 30% gave very good and 3.3% gave good.
  • 13.3% of the participants gave excellent rating to the topics covered, 20% gave Very good while 13.1% gave good.
  • 76.7% of the participants gave excellent rating to the presentation of the skill stations, 16.7% gave very good and 6.7% gave good.
  • 63.3% of the participants gave excellent rating to the interaction during the session, 33.3% gave very good and 3.4% gave good.
  • 60% of the participants gave excellent rating to the clinical application of the course, 36.7% of them gave very good and 3.3% gave good.
  • 63.3% of the participants gave excellent rating on the knowledge and attitude of the facilitators, 33.3% gave very good and 3.4% gave good.

Credentialling:

The participants have been awarded with BLS & ACLS Provider E-Card by American Heart Association upon successful completion of the course valid for a period of two years.
