7.       SC/ST/OBC/MBC certificate for candidate with less than 45% marks in PCBE

8.       Domicile / PRC certificate (for North East category)

9.       Two set of Xerox copies of all the above mentioned  documents

10.   For PCBSc. in addition to above document

1.       Demand Draft of Rs. 1,39,800/- (Rs. 75,800/- as tuition, other fees + Rs. 64,000/- for hostel, mess, utility charges) in favour of “Sikkim Manipal Institute of Medical Sciences” payable at “Gangtok”.
